Nurse Practitioner New

We are hiring a Nurse Practitioner to join our interdisciplinary team conducting assessments in a Senior Community Care (SCC) program in Miami-Dade County. This program provides comprehensive medical services to high-risk health plan members, ensuring they receive the right care at the right time.

Schedule: Monday–Friday, 8 AM – 5 PM

Key Responsibilities:

• Conduct comprehensive assessments of patients
• Manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation of patient conditions (ICD-10)
• Lead advanced illness and care planning discussions

Qualifications:

• Active and unrestricted NP in Florida (or ability to obtain before start date)
• NP: Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
• Board Certification:
• NP: Certified through AANP or ANCC (specialties include Family, Adult, Gerontology, or Acute Care)
• Current, active DEA licensure/prescriptive authority
• Ability to perform physical duties, including lifting a 30-pound bag, navigating stairs, and kneeling for assessments
• Reliable transportation for travel between six facilities during shift
• Ability to obtain collaborative practice agreement, if applicable in Florida
• Full COVID-19 vaccination (per company and client regulations)

Preferred Qualifications:

• 1+ year of clinical experience (long-term care preferred)
• Knowledge of geriatrics, chronic illness, and advanced care planning

Additional Details:

• No weekends, holidays, or on-call duties
