Position Description

Pride Health is hiring a Nurse Practitioner in Miami-Dade County, FL! (Contract to hire)

Work between 6 Skilled Nursing Facilities in Miami-Dade County

Schedule: Monday-Friday (8am-5pm)

Contract: Contract to hire

Pay Rate: $66-69/hour

Specific Duties:

  • Conduct comprehensive assessments
  • Effectively manage acute and chronic medical and behavioral conditions
  • Ensure accurate and complete documentation, including ICD 10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education

Must have qualifications/experience:

  • Nurse Practitioner or Physician Assistant
  • NP Certified in one of the following (FNP, ANP, GNP)
  • Access to transportation
  • 1+ year of experience (LTC, SNF, Geriatric preferred)
